Governing Across the Continuum of Care & Cost of Care by Case
(Huron Healthcare)

Health care organizations face a dual reimbursement world with fee-for-service revenue under increasing pressure, while the percentage of revenue from value-based payment models grows. This shift requires a detailed understanding of total cost of care by bundle and by DRG and the ability to use that information to make decisions – at the management level and at the board level – and align the organization around lower cost, higher quality care. This session introduces an innovative performance management model to identify true costs across the continuum of care and drive evidence-based decision-making.

Major steps to achieve a data-driven transformation to value-based care include:

  • Aligning the organization around a strategy, plan and metrics for succeeding in a value-based care environment
  • Building a culture of accountability and evidence-based decision-making
  • Utilizing technology to support complex business needs and enable informed decisions
  • Implementing data-driven solutions across the continuum of care

These steps enable leaders to understand and improve the total cost of care across an increasingly complex continuum and allow organizations to optimally manage value and performance across the enterprise.

Empowering a Higher Level of Health Insurance Literacy
(Employee Family Protection, Inc).

In a post-ACA era empowering employees to gain a higher level of health insurance literacy is becoming more and more important to the overall health of an organizations bottom line. Employees understanding and being able to make the decisions necessary to properly navigate the health insurance benefit decision is becoming more and more important with all the options that are available, whether it be an employer sponsored plan or an opportunity to gain valuable protection through a public exchange. It’s becoming increasingly difficult for a family to manage all the aspects of a sound and beneficial benefit planning strategy. Our session on empowering a higher level of health insurance literacy will provide the leaders of the organizations with industry trends on what is important to their most important asset, their employees. Increasing the level of awareness around a better health insurance literacy rate will drive efficiency and a better return on investment.

Preferred SNF Networks: A Key Strategy to Flourishing in a Value-Driven Market
(Health Dimensions Group)

The Affordable Care Act has created a shifting landscape toward value-based care with new penalties, mandates, and incentives such as readmission and Medicare spend per beneficiary penalties, bundling programs, as well as ACO and Medicare Advantage growth. Success in this value-based environment requires managing patients across the entire care continuum, prompting hospitals to work more closely with post-acute providers. An important tactic for responding to these changes is the establishment of narrow networks of post-acute partners to encourage improvements in quality of care, patient transitions, and highly efficient services. Preferred SNF network partnerships are key to successfully navigating today’s transition and flourishing in the future for both acute and post-acute providers.
