2022 Registration Process for Participants and Guests
Registrations are now being accepted for the 2022 program. To register, please call Adele Gianino, CHA director of meetings and travel, at (314) 253-3443, or Dee Walsh, CHA meeting planner, at (314) 253-3439. Please leave a voicemail if no one is unavailable.
IMPORTANT REGISTRATION NOTICES:
- Space is limited to approximately 50 participants.
- Those 50 persons and their guests will be registered on a first-come, first-served basis by calling Adele Gianino.
- The deadline to register is Nov. 12, 2021, however, because of the popularity of this program, we suggest you make your plans and register as soon as possible.
- Once the 50 spots have been filled, CHA will compile a wait-list.
PLEASE HAVE THE FOLLOWING INFORMATION AVAILABLE AT THE TIME YOU CONTACT ADELE GIANINO:
- Contact person — if more than one person attending from the organization
- Name and contact information.
- For the participant(s):
- Name, with all relevant prefixes and suffixes
- Preferred name for badge
- Title in health care (example: trustee, CEO, etc.)
- System or facility name and city/state/zip
- Personal email address
- For a guest for each participant:
- Name, with all relevant prefixes and/or suffixes
- Preferred name for badge
- Personal email address
- If the guest is rooming with a participant or requires a separate room
IMPORTANT PASSPORT INFORMATION
While not required at the time of registration, a passport number and exact name included on the passport, or a copy of the application for a passport for those who do not yet have one, will be required soon after registration. For those who do not yet have a passport or need to renew — participants or guests — please note that there is a 10 to 12 week wait for application and receipt.
Please note: Your passport must be valid for at least three months beyond your planned return date from the European Union.

IMPORTANT COVID INFORMATION
Please note that travel requirements to Italy and other countries in the European Union is fluid. It is highly recommended that all Registered Participants and Guests be fully vaccinated for the coronavirus. Currently, Italy is requiring proof of vaccination or a negative coronavirus test prior to entering the country. Masks are still required for indoor settings.
We highly recommend that participants and guests stay informed by following the guidelines for Italy which can be found on the sites for the US Embassy & Consulates in Italy – https://it.usembassy.gov and Travel.State.Gov, at https://travel.state.gov/content/travel/en/international-travel/International-Travel-Country-Information-Pages/Italy.html. Please note that if you plan additional travel to other countries in the EU before or after the CHA program, the COVID guidelines may vary.
