Webinar: A Focus on Health Equity and Coverage
The Institute for Diversity in Health Management in collaboration with the AHA's Hospitals in Pursuit of Excellence, Equity of Care and Hospital Engagement Network 2.0, will host a webinar focused on health equity and coverage. Cara James will discuss CMS's "From Coverage to Care" initiative, which is designed to help Americans understand health coverage options and make the most of primary care and preventive services available to them. April Diversity Dialogue
The title of the Institute for Diversity's April Diversity Dialogue is "A Journey Toward Equity: Tales of the Wild West." Equity is inherent in the nature of health care supported with the desire to do the right thing and good by all served. But the truth is, inequity is blind. It is subtle. It is unintentional. No one wants to believe their health system delivers care with inequities. While the journey towards equity can be daunting, even paralyzing, starting the journey towards equity doesn't mean investing high dollars. It does, however, require investing in and understanding the true value of using a disparity and equity lens when viewing quality issues in health care. This session uses a case study method to share one health system's journey from equity blind to equity perceptive and will take place April 27, from 10 to 11 a.m. PT, noon to 1 p.m. CT, 1 to 2 p.m. ET. The Healthcare Quality and Equity Action Forum
The Disparities Solutions Center at Massachusetts General Hospital will host the 2016 Healthcare Quality and Equity Action Forum Sept. 29 - 30 at the Seaport Boston Hotel. The Forum will provide essential strategies to improve quality, achieve equity and deliver high-value health care to diverse populations. Leaders from multiple disciplines and health care organizations who work in the areas of quality and safety; disparities, diversity and equity; health policy; and health care design and delivery, as well as anyone interested in these fields, will find the Forum to be a unique and worthwhile experience. Office of Minority Health: Chartbook on Health Care for Blacks
The Agency for Healthcare Research and Quality (AHRQ) has released the Black Health Care Chartbook as part of a family of documents and tools that support the National Healthcare Quality and Disparities Report (QDR). The chartbook includes a summary of trends across measures of black/African American health care from the QDR. A PowerPoint version is also available that users can download for presentations. CMS Guide to Preventing Readmissions among Racially and Ethnically Diverse Medicare Beneficiaries
This Guide was developed in collaboration with the Disparities Solutions Center at Massachusetts General Hospital and the National Opinion Research Center (NORC) at the University of Chicago. It is part of the Centers for Medicare & Medicaid Services (CMS) Equity Plan for Improving Quality in Medicare, and is designed to assist hospital leaders and stakeholders focused on quality, safety and care redesign in identifying root causes and solutions for preventing avoidable readmissions among racially and ethnically diverse Medicare beneficiaries. CMS Interactive Mapping Medicare Disparities Tool
The CMS Office of Minority Health (CMS OMH) released a new interactive map to increase understanding of geographic disparities in chronic disease among Medicare beneficiaries. The Mapping Medicare Disparities (MMD) Tool identifies disparities in health outcomes, utilization, and spending by race and ethnicity and geographic location. View the map.
CDC: Strategies for Reducing Health Disparities Selected CDC-Sponsored Interventions, United States 2016
Reducing health disparities is a major goal of public health. Despite the persistence of disparities, progress is being made. Since 2011, The Strategies for Reducing Health Disparities report has highlighted effective public health programs that have demonstrably reduced disparities. This report describes interventions that have demonstrated effectiveness or showed promise for decreasing health disparities. Access the report.
America's Health Rankings Spotlight Report
The America's Health Rankings Spotlight Prevention report was released in partnership with the American College of Preventive Medicine. This report takes an in-depth look at the status of clinical disease prevention across the country and the inequities within subpopulations. The objective of the report is to drive awareness and understanding about the roles of key clinical preventive services and interventions, access to health care, immunizations and chronic disease prevention in improving the health of individuals and our communities. Kaiser Family Foundation Issue Brief: Medicaid and American Indians and Alaska Natives
This brief provides an overview of the health needs of American Indians and Alaska Natives, discusses the role of Medicaid and the potential impact of the Medicaid expansion for this population, and summarizes new guidance from the CMS that expands the scope of Medicaid services provided to American Indians and Alaska Natives that may qualify for 100 percent federal match. The Disparities Solutions Center at Massachusetts General Hospital: Going Beyond REaL Data Collection: Collecting Social Determinants of Health

Kirsten Bibbins-Domingo, Ph.D., MD, MAS, director of the UCSF Center for Vulnerable Populations at San Francisco General Hospital, discussed the Institute of Medicine (IOM) report, "Capturing Social and Behavior Health Domains and Measures in Electronic Health Records" and its recommendations. Aswita Tan-McGrory, MBA, MSPH, deputy director of the Disparities Solutions Center, discussed the implementation of collection data on social determinants of health and what we are seeing in the field. IOM Community Based Solutions to Promote Health Equity in the United States
The IOM's Committee on Community Based Solutions to Promote Health Equity in the United States was established to assist The Robert Wood Foundation's Culture of Health Initiative in delineating causes of and solutions to health inequities in the United States. The committee has held two public meetings with presentations on topics related to health disparities and community programs designed to achieve health equity.

IOM Framing the Dialogue on Race and Ethnicity to Advance Health Equity: A Workshop
There are several areas of public health where the evidence for improving the underlying factors that shape poor health outcomes of vulnerable populations is robust, but is not effectively communicated and translated to inform the public and decision makers at various levels of government and in the private sector. This workshop by the IOM's Roundtable on Population Health Improvement explored different strategies for framing the dialogue on equity and inequity among racial and ethnic groups to advance population health equity.
