At Saint Alphonsus Regional Medical Center, in Boise, Idaho, the CARE initiative ensures that health advisors and interpreters are available for its large number of immigrant and refugee maternity patients who might come from Iraq, the Sudan, Somali or elsewhere.

Health coaches, true peer advisers, frequently serve as health care advocates and case coordinators, meeting with CARE families to ensure that they have applied for health coverage, understand appointment scheduling and other expectations, and know what to expect during medical appointments. They also help ensure that patients and families understand when — and how often — to access routine preventive care, among other things.

Participating with the patients and their families all the way through patient follow-up with specialists and through the search for additional resources for other health needs, the health advisor's final task is helping the patient transition into an accepting mainstream primary care clinic following their participation with CARE.
