2008 Assembly Registration Fees:

• Member Registrants ? $750
  Member Registrants are persons employed by or serving in governance of a CHA-member organization and that organization is paying or reimbursing them for their registration fees.

• Ministry Partner Registrants ? $825
  Ministry Partner Registrants are persons employed by or serving in governance of an organization, other than a CHA member, that is listed in the Official Catholic Directory, and that organization is paying or reimbursing them for their registration fees.

• Sponsored Registrants ? $1,125
  Sponsored Registrants are persons who are not Member Registrants or Ministry Partner Registrants. In order to attend the 2008 Catholic Health Assembly, Sponsored Registrants will need to be endorsed by CHA or a CHA-member organization. Due to these requirements, Sponsored Registrants will not be able to register online. To register, please download, fill out, and return the Assembly Registration Form (.pdf) and Form of Endorsement Letter (.doc).

  Note: CHA shall have the right in its sole discretion to determine if a registrant meets the above criteria for registration for the 2008 Catholic Health Assembly. If registration is declined, CHA will refund the fees paid.

  $150 ? CHA Member; $165 ? Ministry Partner; $180 ? Nonmember

• Retention of Valued Employees: Love Them or Lose Them? $150
  A pre-assembly workshop presented by Avila Institute of Gerontology, Inc.

Cancellation Policy
Contact CHARegistrar@chausa.org should you be unable to attend this program. Cancellations received by Friday, May 30, will be assessed a cancellation fee of $100; after that date, no refund will be given. Registration fees are not transferable to another CHA meeting.

There is no additional charge for substitutes; however, please provide substitute names by Friday, May 30.
