CHA developed the guide in collaboration with physicians, nurses, theologians and ethicists within Catholic health care and with the Supportive Care Coalition, a national coalition of Catholic health ministries that advances excellence in palliative care. Although the guide reflects the perspective of Catholic tradition, the information can be helpful to people from any religious tradition. It is part of a series of publications from CHA to assist patients, families, clinicians, caregivers and others with decisions about palliative care, hospice care, advance directives, serious illness and end-of-life care.

The handy design will easily fit into displays and are convenient handouts to patients, families and caregivers in acute care hospitals, long-term care facilities, physician offices, clinics, parishes and community centers.
