Building on the conversations started at CHA’s 2010 Catholic Health Strategy Summit, this webinar explored how faith-based health care providers, throughout the continuum of care, can better integrate care of older and chronically ill persons through community-based programs. Health policy writer, Howard Gleckman presents an overview of provisions of the Affordable Care Act that are driving care integration and provides an overview of community–based integrated programs. Nationally recognized leader on how religious communities can impact health and wholeness, Gary Gunderson describes the Memphis Community Health Network. This is a model for connecting faith-communities and health care providers in order to improve the health of communities.
