The Pre-Assembly Sponsorship Forum is an opportunity to continue the ongoing conversation about sponsorship and its relationship to governance. The meeting will consist of presentations concerning the history, theology and evolving models of sponsorship. There will be time to discuss topics including the sponsor-governance relationship, mission leader-sponsor relationship, identifying and forming sponsor members and essential sponsor duties. This forum is designed for sponsor members and board members seeking to clarify their roles and enhance their relationship. CEOs and senior mission leaders will also benefit from the discussions concerning the sponsor relationships and the evolving role of sponsor.

All participants are invited to a special joint reception with Tomorrow's Leaders honorees on Saturday evening at the end of the program session.

CERTIFICATE OF ATTENDANCE

CHA will provide all participants with a Certificate of Attendance indicating hours upon submission of a Record of Attendance.

Immediately following the close of Assembly, all participants will receive an email with a link to the program evaluation survey and the Record of Attendance form. To receive an individualized Certificate of Attendance, attendees must complete and submit the Record of Attendance form to CHA.

Please note CHA does not offer Continuing Education Units (CEUs) for Assembly or any programs.

Participants may submit Certificates of Attendance to relevant organizations that accept proof of contact hours as credit. These include:

  • American College of Healthcare Executives: American College of Healthcare Executives (ACHE) members may self-report qualifying CEUs using the contact hour information provided on their CHA Certificate of Attendance. To self-report, log in as a member on the ACHE site (www.ache.org); use the link on the left-hand side to self-report applicable contact hours for CEU credit.
  • NACC Board Certified Chaplains: National Association of Catholic Chaplains (NACC) board certified chaplain members may self-report their continuing education hours (CEH's) using the contact hours information provided on their CHA Certificate of Attendance.

CANCELLATIONS

Pre-Assembly Sponsorship Forum
Cancellation and refund requests must be made in writing (mail or email). Phone cancellations will not be accepted.

Submit all requests to CHA Service Center via email at servicecenter@chausa.org or mail to CHA-Service Center, 4455 Woodson Road, St. Louis, MO  63134. CHA regrets that refunds will not be given for no-shows; however, substitutions are gladly accepted.

Cancellations received by May 1: A refund of the full registration fee, minus a $100 cancellation fee, will be given for cancellations received by May 1, 2020.

Cancellations received between May 2 and May 15: A refund of 50% of the registration fee will be given for cancellations received between May 2 and May 15, 2020.

No refunds will be granted for requests made after May 15, 2020.
