CHA's annual Physician Leader Forum brings together physician leaders and mission leaders who work directly in physician formation, from across the ministry to address timely issues clinical leaders in Catholic health care are facing. For 2019, CHA has scheduled the event to coincide with its Assembly 2019 in Dallas at the suggestion of CHA's Physician Leader Advisory Committee. The focus will be on developing the competencies of physician leaders in Catholic health care. One of those competencies is knowledge of Catholic moral teaching and the ability to speak to physician colleagues about the Ethical and Religious Directives for Catholic Health Care Services. This will be a topic developed during the forum. In addition to this Sunday morning content devoted to physician leaders registered for the forum, there will be a private Physician Luncheon held on Monday, June 10, followed by innovation sessions, with several specifically oriented to physician leaders.

Certificate of Attendance

CHA will provide all participants with a Certificate of Attendance indicating program contact hours upon submission of a Record of Attendance.

Immediately following the close of Assembly, all participants will receive an email with a link to the program evaluation survey and the Record of Attendance form. To receive an individualized Certificate of Attendance, attendees must complete and submit the Record of Attendance form to CHA.

Please note CHA does not offer Continuing Education Units (CEUs) for Assembly or any programs.

Participants may submit Certificates of Attendance to relevant organizations that accept proof of contact hours as credit. These include:

  • American College of Healthcare Executives: American College of Healthcare Executives (ACHE) members may self-report qualifying CEUs using the contact hour information provided on their CHA Certificate of Attendance. To self-report, log in as a member on the ACHE site (; use the link on the left-hand side to self-report applicable contact hours for CEU credit.

  • NACC Board Certified Chaplains: National Association of Catholic Chaplains (NACC) board certified chaplain members may self-report their continuing education hours (CEH's) using the contact hours information provided on their CHA Certificate of Attendance.

Cancellation Policy

Pre-Assembly Physician Leader Forum
Cancellation and refund requests must be made in writing (mail or email). Phone cancellations will not be accepted.

Submit all requests to CHA Service Center via email at or mail to CHA-Service Center, 4455 Woodson Road, St. Louis, MO  63134. CHA regrets that refunds will not be given for no-shows; however, substitutions are gladly accepted.

Cancellations received through April 1: A refund of the full registration fee, minus a $100 cancellation fee, will be given for cancellations received through April 1, 2019.

Cancellations received between April 2 and May 17:  A refund of 50% of the registration fee will be given for cancellations received between April 2 and May 17, 2019.

No refunds will be granted for requests made after May 17, 2019.
