This seminar will bring together mission and formation leaders from across the continuum of care to gain timely insights into the topics, trends, issues and opportunities of mission integration and formation in the health ministry. The focus of the seminar this year will be on the role of the mission leader in formation. While some systems are developing the role of formation leaders, there continues to be a need for mission leaders to advance in facilitation and presentation skills. Through presentations and dialogue, participants will grow in their awareness of the resources that are currently available and/or being developed for mission and formation leaders.

While the agenda and speakers are still being finalized, please note that in 2019 this will be a Saturday-only event. However, there are two programs on Sunday morning that may be of interest.

Certificate of Attendance

CHA will provide all participants with a Certificate of Attendance indicating hours upon submission of a Record of Attendance.

Immediately following the close of Assembly, all participants will receive an email with a link to the program evaluation survey and the Record of Attendance form. To receive an individualized Certificate of Attendance, attendees must complete and submit the Record of Attendance form to CHA.

Please note CHA does not offer Continuing Education Units (CEUs) for Assembly or any programs.

Participants may submit Certificates of Attendance to relevant organizations that accept proof of contact hours as credit. These include:

  • American College of Healthcare Executives: American College of Healthcare Executives (ACHE) members may self-report qualifying CEUs using the contact hour information provided on their CHA Certificate of Attendance. To self-report, log in as a member on the ACHE site (www.ache.org); use the link on the left-hand side to self-report applicable contact hours for CEU credit.

  • NACC Board Certified Chaplains: National Association of Catholic Chaplains (NACC) board certified chaplain members may self-report their continuing education hours (CEH's) using the contact hours information provided on their CHA Certificate of Attendance.

Cancellation Policy

Pre-Assembly Mission Leader Seminar
Cancellation and refund requests must be made in writing (mail or email). Phone cancellations will not be accepted.

Submit all requests to CHA Service Center via email at servicecenter@chausa.org or mail to CHA-Service Center, 4455 Woodson Road, St. Louis, MO  63134. CHA regrets that refunds will not be given for no-shows; however, substitutions are gladly accepted.

Cancellations received by April 1: A refund of the full registration fee, minus a $50 cancellation fee, will be given for cancellations received by April 1, 2019.

Cancellations received between April 1 and May 17: A refund of 50% of the registration fee will be given for cancellations received between April 1 and May 17, 2019.

No refunds will be granted for requests made after May 17, 2019.
