Assembly 2019 Registration Information and Fees

NO PRICE INCREASE IN 2019!

For the fourth year in a row, Assembly registration fees for CHA members will remain at the previous year's pricing!

Included in Assembly Registration
A registration for the main Assembly includes conference programming and numerous networking opportunities throughout the event, including: exhibitions in the exhibit hall; liturgy services; all food and beverage events (lunch and Welcome Reception on Sunday; breakfast, lunch, Awards Reception, Banquet and After Party on Monday; breakfast on Tuesday).

CHA Member Registrant

Registrants who are not CHA members may attend the 2019 Catholic Health Assembly only if endorsed by CHA or a CHA-member organization. After an individual completes the registration process, CHA will contact her/him to confirm that s/he has the proper endorsement.

Note: CHA shall have the right in its sole discretion to determine if a registrant meets the above criteria for registration for the 2019 Catholic Health Assembly. If registration is declined, CHA will refund the fees paid.

HOTEL RESERVATIONS

To get the discounted conference rate at the Sheraton Dallas Hotel, you must first register for the Assembly. At the end of the registration process, you will get a link that takes you to the housing page where you can reserve your sleep room. ADDITIONAL OPTIONS

Personal Guest Registration — $295 per person
A Personal Guest Registration is an option for the guests of registered assembly attendees who wish to attend general session programming and visit the exhibit hall, as well as the following food and beverage events: lunch and Welcome Reception on Sunday; breakfast, lunch, Awards Reception, Banquet and After Party on Monday; breakfast on Tuesday. (If a guest purchases a Personal Guest badge, there is no need for that same guest to purchase an Evening Events badge.)

A Personal Guest Registration is not applicable for professional colleagues from the same organization as the Assembly registrant, nor does it allow admittance to Innovation Forum programming. The word "Guest" will be clearly noted on the badge.

Evening Events Registration — $95 per person
An Evening Events Registration is designed for guests of Assembly registrants who wish to attend ONLY the following evening social events: Welcome Reception on Sunday; Awards Reception, Banquet and After Party on Monday. (If a guest purchases an Evening Events badge, there is no need for that same guest to also purchase a Personal Guest badge.)

An Evening Events Registration does not allow admittance to any other Assembly programming, activities, breakfasts or lunches. The word "Guest" will be clearly noted on the badge.


CANCELLATION POLICY

Cancellation and refund requests must be made in writing (mail or email). Phone cancellations will not be accepted.

Submit all requests to CHA Service Center via email at servicecenter@chausa.org or mail to: CHA-Service Center, 4455 Woodson Road, St. Louis, MO  63134.

CHA regrets that refunds will not be given for no-shows; however, substitutions are gladly accepted.

Cancellations received through April 1:  A refund of the full registration fee, minus a $100 cancellation fee, will be given for cancellations received through April 1, 2019.

Cancellations received between April 2 and May 17:  A refund of 50% of the registration fee will be given for cancellations received between April 2 and May 17, 2019.

No refunds will be granted for requests made after May 17, 2019.



NOTICES

During the Assembly, you may be photographed and/or recorded by CHA staff or its designated contractors. By your attendance or participation at the Assembly or affinity event, you agree that CHA may photograph, copy, record and/or distribute in any such form your participation and involvement in any Assembly session, activity or event. No persons or organizations other than CHA or its designated contractors may photograph, audiotape or videotape any portion of the Assembly without prior written permission of CHA. CHA reserves the right to change speakers or to modify program content.

CHA will provide an Assembly registrant list to "Friends of the Assembly" sponsoring organizations. The generous support from these sponsors helps keep the registration fees low for CHA-member attendees. CHA will share only registrant names, mailing addresses and email addresses (as indicated on the registration forms). Sponsors agree to use the registrant list only once.

CHA shall have the right at its sole discretion to decline registration. If registration is declined, CHA will refund the fees paid.
