This article by Howard Gleckman of The Urban Institute was published in the September-October 2012 edition of CHA’s journal, Health Progress. Commissioned by the Catholic Health Association, the article, originally titled New Readmission Laws: Catholic Providers are Positioned to Lead, examines the most common causes of avoidable readmissions from long-term care facilities to hospitals and ways these facilities can reduce these types of readmissions. In it, he concludes that reducing readmissions is not simple and that there is no single, silver-bullet solution. But better training and improved communication are keys to achieving both fewer transitions back to the hospital and, most importantly, better patient outcomes. He surmises that for mission-based systems, the goal of reducing readmissions goes beyond payment rules or government regulations, and that it is not only good business practice, but the right thing to do.
