2015 Assembly Registration


SPECIAL CENTENNIAL SAVINGS — Exclusively for CHA Members through April 17!

As part of CHA's 100th anniversary celebration, CHA members who purchase a full assembly registration by April 17 will get a $100 credit per night for up to a two-night stay (for a potential maximum savings of $200) at the Marriott Wardman Park, the site of the 2015 Assembly. This special Centennial Savings is good for up to two nights, between June 6 and 8, 2015. Your Centennial savings will appear as a credit on your final hotel bill.

2015 ASSEMBLY REGISTRATION FEES

In addition to conference programming, an Assembly registration includes two breakfasts, two lunches, two cocktail receptions, the annual Awards Banquet and After Party, networking and activities in the exhibit hall and liturgy services.

Member Registrants

  • Through April 17 — $995 (Register by April 17 at this discounted rate PLUS receive a special Centennial savings of $100 per night for up to a two-night stay — for a maximum credit of $200 at the assembly hotel — for a potential combined savings of $400 on registration and hotel!)
  • April 18 to June 5 — $1,195
  • Onsite registration — $1,395

Registrants who are not CHA members may attend the Catholic Health Assembly. All registrations will be reviewed by CHA prior to being accepted. You will be contacted directly should there be any questions about your registration.

Note: CHA shall have the right in its sole discretion to determine if a registrant meets the above criteria for registration for the 2015 Catholic Health Assembly. ADDITIONAL OPTIONS

PERSONAL GUEST REGISTRATION — $275 per person
A Personal Guest Registration is an option for the guests of registered assembly attendees who wish to attend general session programming and visit the exhibit hall, as well as the following food and beverage events: lunch and welcome reception on Sunday; breakfast, lunch, annual Awards Banquet and After Party on Monday; and breakfast on Tuesday.

  • A Personal Guest Registration is not applicable for professional colleagues from the same organization as the assembly registrant, nor does it allow admittance to Innovation Forum programming. The word "Guest" will be clearly noted on the badge.
  • Personal Guest Registration fees are not refundable.

EVENING EVENTS REGISTRATION — $85 per person
An Evening Events Registration is designed for guests of assembly registrants who wish to attend ONLY the following evening social events: welcome reception on Sunday; reception, annual Awards Banquet and After Party on Monday.

  • An Evening Events Registration does not allow admittance to any other assembly programming or activities. The word "Guest" will be clearly noted on the badge.
  • Evening Events Registration fees are not refundable.


CANCELLATION POLICY

Cancellation and refund requests must be made in writing (mail or email).  Phone cancellations will not be accepted. Submit all requests to CHA Service Center via email at servicecenter@chausa.org or mail to CHA-Service Center, 4455 Woodson Road, St. Louis, MO  63134.

CHA regrets that refunds will not be given for no-shows; however, substitutions are gladly accepted.

Cancellations received by April 17:  A refund of the full 2015 Assembly registration fee, minus a $100 cancellation fee, will be given for assembly cancellations received by April 17, 2015. Cancellations for either the Pre-Assembly Avila program or Pre-Assembly Community Benefit program will receive a refund of the full registration fee for either of those pre-assembly events, minus a $50 cancellation fee. Cancellations for Personal Guests and Evening Events will not receive a refund.

Cancellations received between April 18 and May 14:  A refund of 50% of the registration fee will be given for cancellations received between April 18 - May 14, 2015.

No refunds will be granted for requests made after May 14, 2015.


HOTEL RESERVATIONS

NOTICES

During the assembly, you may be photographed and/or recorded by CHA staff or its designated contractors. By your attendance or participation at the assembly or affinity event, you agree that CHA may photograph, copy, record and/or distribute in any such form your participation and involvement in any assembly session, activity or event. No persons or organizations other than CHA or its designated contractors may photograph, audiotape or videotape any portion of the assembly without prior written permission of CHA. CHA reserves the right to change speakers or to modify program content.

CHA will provide an assembly registrant list to "Friends of the Assembly" sponsoring organizations. The generous support from these sponsors helps keep the registration fees low for CHA-member attendees. CHA will share only registrant names, mailing addresses and email addresses (as indicated on the registration forms). Sponsors agree to use the registrant list only once.

CHA shall have the right at its sole discretion to decline registration. If registration is declined, CHA will refund the fees paid.
