Partnerships Active in Communities to Achieve Health Equity (PAC) is a program of Mercy St. Vincent Medical Center's Family Care Center. PAC is a collaborative program that aims to improve the health of racial and ethnic minority adults in Toledo/Lucas County, Ohio, by addressing disparities and gaps in diabetes prevention and care.

The project identifies minority adults who do not have a medical home, and connects them to health care and support services in order to improve health outcomes for those at highest risk. PAC uses an integrated health care model that combines diabetes prevention, intervention and chronic disease management at five adult health care sites that serve the minority community.

Program participants receive screenings and education for diabetes, medication management, monitoring of weight, nutrition education and counseling, fitness education, behavioral health, and case management and peer support. All of the services are provided at multiple sites so they are convenient, accessible, better utilized, and culturally appropriate.
