Program Site/Accommodations
The program will be held at Sheraton Gateway Suites O'Hare, 6501 North Mannheim Road, Rosemont, IL 60018. A block of guest rooms has been reserved at the special group rate of $169 per night for a single-double.
To reserve your room at the special group rate, contact the hotel directly at (888) 627-8117. Your reservation must be received by Monday, April 21, to receive the discounted rate. Be sure to mention the American Hospital Association when reserving your room. Check-in time is 3 p.m. and checkout time is noon. The hotel maintains a 24-hour cancellation policy for individual guest room reservations.
Registration Fees
There are three costs for this program depending on the registrant:
CHA and AHA Members: $500
Member Registrants are persons employed by or serving in governance of a CHA-member organization, or an AHA member-member organization and that organization is paying or reimbursing them for their registration fees.
Ministry Partner: $550
Ministry Partner Registrants are persons employed by or serving in governance of an organization, other than a CHA and AHA member, that is listed in the Official Catholic Directory, and that organization is paying or reimbursing them for their registration fees.
Other Registrant: $600
Other Registrants are persons who are not Member Registrants or Ministry Partner Registrants.
The registration fee includes one reception, two continental breakfasts, one lunch, refreshment breaks, and program materials.

Program registration is made through CHA. Room reservations need to be made separately through Sheraton Gateway Suites O'Hare, Chicago. Registering for the program does not guarantee room availability, so please make your reservations today.

Airport Transportation
The Sheraton (two miles from O'Hare) provides a complimentary shuttle bus at Gate 3. The shuttle makes pick-ups every 20 minutes.
Air Travel
For travel arrangements, please call CHA's travel provider, American Express, at (314) 253-3442, or e-mail your request and identify yourself as a CHA System Mission Leadership Forum attendee. Registrants who purchase restrictive airfares are responsible for all penalties.
Cancellation
To cancel your registration, contact CHARegistrar. To qualify for a refund, minus a $50 administrative fee, your cancellation must be received no later than April 21, 2008. No refunds will be given after April 21. Substitutes can be sent at no charge. Contact CHARegistrar if you need registration or cancellation assistance.
