    REGISTRATION FEES

    CHA-Member Registrants: $ 450 ($380 through July 16)
    Member Registrants are persons employed by or serving in governance of a CHA-member organization and that organization is paying or reimbursing them for their registration fees.

    An early registration discount is available to CHA members only through July 16, 2012. This makes the registration cost $380 through July 16.

    Other Registrants: $540
    Other Registrants are persons who are not Member Registrants.

    PROGRAM/SITE ACCOMMODATIONS

    CHA's Community Benefit 101 program will be held at the Chase Park Plaza Hotel, 212-232 North Kingshighway Blvd., St. Louis, MO 63108. A block of guest rooms has been reserved at the special group rate of $145 for a deluxe room which includes WiFi. To reserve your room at the special group rate, please contact the hotel directly at (314) 633-3000 or (877) 587-2427.

    Please be sure to call to make your room reservation, and mention CHA, in order to receive our discounted rate.

    Your reservation must be received by Oct. 1, 2012, to receive the discounted rate. Reservations may be made Monday through Friday from 8 a.m. to 8 p.m. CT. Be sure to mention the CHA Community Benefit 101 when reserving your room. Check-in time is after 4 p.m. and checkout time is before noon. Cancellation of individual room reservations must be made 72 hours (three business days) prior to your scheduled day of arrival to avoid being charged for the first night stay.

    Room reservations need to be made separately through the Chase Park Plaza. Registering for the program does not guarantee room availability.

    AIRPORT TRANSPORTATION

    • Taxi fare from the Lambert — St. Louis International Airport is approximately $38 each way — please allow 30 to 35 minutes.
    • MetroLink light rail from the airport to the Central West End stop is $3 one way. Rates are subject to change. The walk from the MetroLink to the hotel is approximately 12 minutes (five blocks). When leaving the rail station, go up the stairs, turn right, and start walking north on Euclid Avenue (approximately seven minutes). When you come to Lindell Boulevard, turn left (west) and walk one block to the Chase Park Plaza Hotel.

    AIR TRAVEL

    For travel arrangements, please call CHA's travel provider, American Express, at (314) 253-3442, or email your request and identify yourself as a CHA Community Benefit 101 attendee. Registrants who purchase restrictive airfares are responsible for all penalties.

    CONTINUING EDUCATION/CERTIFICATES OF ATTENDANCE

    ACHE: The Catholic Health Association is authorized to award pre-approved Category II (non-ACHE) continuing education credit for this program towards advancement, recertification, or reappointment in the American College of Healthcare Executives. Participants in this program wishing to have the continued education hours applied toward Category II credit should indicate their attendance when submitting application to the American College of Healthcare Executives for advancement or recertification.

    CANCELLATION POLICY

    To cancel your registration, contact CHARegistrar@chausa.org. To qualify for a refund, your cancellation must be received no later than Oct. 9, 2012.  There is no charge for sending a substitute.

    NOTICES

    During the meeting, you may be photographed, videotaped, and/or recorded by CHA staff or its designated contractor. By your attendance or participation at the meeting, you agree that CHA may photograph, copy, record, and/or distribute in any such form your participation and involvement in any meeting session, activity or event. No persons or organizations other than CHA may photograph, audiotape or videotape any portion of the meeting without prior written permission of CHA.

    The right to change speakers or to modify program content is reserved by CHA.
