OVERVIEW
Not-for-profit health care organizations were founded to meet the most pressing health needs of their communities. A vital part of this mission is to effectively assess community health needs and develop actionable plans to address those needs. These processes — assessment and planning — were recently mandated for tax-exempt hospitals in the Affordable Care Act and in guidance from the Internal Revenue Service.
This program will highlight both the mission and legal mandates for community health needs assessments and implementation strategies and present information on the basic steps of assessment and planning: accessing and using public health and other data, translating data into useful information, collaborating with community partners, setting priorities, and building on organization and community assets to develop plans for community health improvement.

OBJECTIVES
As a result of this program, participants will be able to:
- Describe the legal requirements for community health needs assessments and implementation strategies.
- Identify and evaluate the applicability of available assessment and planning tools and resources.
- Build on leading practices that are being successfully implemented by public health agencies, hospitals and health systems.

- Community members and groups — How to involve community partners in assessment and planning.
