    Assessing & Addressing Community Health Needs

    A National Conference Sponsored by CHA and VHA Inc.

    OVERVIEW

    Not-for-profit health care organizations were founded to meet the most pressing health needs of their communities. A vital part of this mission is to effectively assess community health needs and develop actionable plans to address those needs. These processes — assessment and planning — were recently mandated for tax-exempt hospitals in the Affordable Care Act and in guidance from the Internal Revenue Service.

    This program will highlight both the mission and legal mandates for community health needs assessments and implementation strategies and present information on the basic steps of assessment and planning: accessing and using public health and other data, translating data into useful information, collaborating with community partners, setting priorities, and building on organization and community assets to develop plans for community health improvement.

    OBJECTIVES

    As a result of this program, participants will be able to:

    • Describe the legal requirements for community health needs assessments and implementation strategies.
    • Identify and evaluate the applicability of available assessment and planning tools and resources.
    • Build on leading practices that are being successfully implemented by public health agencies, hospitals and health systems.

    A discussion of how the mission of our organizations calls us to assess and address the health needs of our communities.
     
    8:45 a.m. Compliance — The Legal Requirements for Assessment and Planning
    An overview of the requirements in the Affordable Care Act and IRS guidance for tax-exempt hospitals to conduct community health needs assessments and develop implementation strategies.
     
    9:45 a.m. Assessment and Planning – Basic Steps and Reports
    Review the basics steps in the assessment and community benefit planning processes and the reports that document them.
     
    10:15 a.m. Break
     
    10:30 a.m. Publicly Accessible Data Sources
    An overview of data sources for assessment and planning, their advantages and disadvantages and tips for how data can be used to engage key partners to improve community health.
     
    11:30 a.m. Assessing and Engaging Local Resources and Assets
    Learn from a panel of experts about:
    • Local health departments — The ways local health departments and hospitals can build on each other's strengths in assessment and planning for community health improvement.
    • Internal resources — How to work with strategic planning, emergency departments and others within the organization to obtain and analyze internal data that can be used to identify community health problems.
    • Community members and groups — How to involve community partners in assessment and planning.
     
    12:30 p.m. Keynote Lunch — Key Concepts in Coalition Building
     
    1:30 p.m. Translating Assessment Data into Community Health Needs
    How to analyze data collected in the assessment to create a list of community health problems and to understand the underlying causes of those problems, including issues related to the social determinants of health.
     
    2:30 p.m. Setting Priorities: Making Decisions
    An overview of established methods and tools for prioritizing community health needs and suggest who needs to be involved in the process.
     
    3:30 p.m. Break
     
    4:00 p.m. Challenges and Opportunities
    Ask the expert presenters and other registrants about first (or next steps) and how to overcome challenges and find new opportunities.

    8:00 a.m. Overview of the Day and Reflection
     
    8:15 a.m. Implementation Strategies: Moving from Data to Action
    How to develop an implementation strategy that builds on community assets, including existing community benefit programs, organizational strengths and community partnerships, to address community health needs.
     
    9:15 a.m. Determining What to Do
    Learn how to select the most appropriate interventions given the organization's strengths and resources, the community's characteristics, resources of community partners and other factors. Sources for evidence-based and promising practices will also be covered.
     
    10:15 a.m. Break
     
    10:45 a.m. Challenges and Opportunities
    Join in an open discussion with presenters who will address potential obstacles to planning and how to address them (budget realities, rural facility challenges, managing expectations and other issues presented by participants).
