Directory of LTC/Continuing Care Facilities and Services
Catholic sponsored programs and services for aged and chronically ill persons includes a full continuum of compassionate quality care. This directory of long-term care and continuing care, developed jointly by CHA and Catholic Charities USA, includes:
- Skilled nursing services in nearly 700 locations
- Nearly 400 assisted living/residential care programs
- Nearly 600 other housing programs, including senior, low-income and special need housing
- Over 170 adult day care programs
- Over 300 home care programs and nearly 400 hospice programs
In addition, the services listed include a rich array of social services and other supportive services for older persons, including respite care, nutrition services, house repair, parish ministry, transportation services, geriatric assessment, case management, and mental health services.
