    2012 Assembly Registration

    In addition to conference programming, an Assembly registration includes breakfasts, lunch, two cocktail receptions and the annual Awards Banquet.
    • Member Registrants — $785 through April 13; $950 after April 13
      CHA-member registrants are persons employed by or serving in governance of a CHA member organization and that organization is paying or reimbursing them for their registration fees.
    • Other Registrants — $1,450
      Registrants who are not CHA members may attend the 2012 Catholic Health Assembly only if endorsed by CHA or a CHA-member organization. After an individual completes the registration process, CHA will contact him/her to confirm that he/she has the proper endorsement.

    Note: CHA shall have the right in its sole discretion to determine if a registrant meets the above criteria for registration for the 2012 Catholic Health Assembly. If registration is declined, CHA will refund the fees paid.

    Post-Assembly Event: 2012 Global Summit
    (Tuesday, June 5)
    As this summit includes part of the assembly sessions, pricing has been determined for both those who attend this as the conclusion of the assembly experience, as well as those who attend the summit only. Registration fees include breakfast, one breakout session for summit attendees and other assembly attendees, a morning session, lunch and two afternoon summit sessions. It will conclude at approximately 5 p.m.

    Assembly Registrants (as add-on to conference): $150
    CHA Members (summit only): $250
    Other Registrants (summit only): $350

    For those who can attend just one day of the assembly, this option will provide full access to programming and events scheduled for that day.

    Additional Options

    • Personal Guest Badge — $250 per person
      A Personal Guest Badge is an option for the guests of registered assembly attendees who wish to attend general session programming and food and beverage events. A Personal Guest Badge will allow access to all general sessions and the exhibit hall, as well as participation in the following food and beverage events: welcome reception on Sunday; breakfast, lunch, reception and awards banquet on Monday; breakfast on Tuesday. (The personal guest badge does not allow admittance to Innovation Forum programming.)
    • Evening Events Badge — $75 per person
      An Evening Events Badge is an option for guests of registered assembly attendees who wish to attend ONLY the evening social events and awards banquet. An Evening Events Badge will allow participation in the following evening events: welcome reception on Sunday; reception and awards banquet on Monday.  (The evening events badge does not allow admittance to any other assembly programming or events.)

    Hotel Reservations

    Cancellation Policy

    To cancel your registration, contact the CHA Service Center at servicecenter@chausa.org or (800) 230-7823. To receive a full refund, you must cancel your registration no later than one week prior to start of the program. No refunds will be given for cancellations received less than one week prior to the start of the program.

    There is no additional charge for substitute registrants; however, please provide substitute names by Friday, May 11.

    Notices

    During the assembly, you may be photographed, videotaped and/or recorded by CHA staff or its designated contractors. By your attendance or participation at the assembly, or affinity event, you agree that CHA may photograph, copy, record and/or distribute in any such form your participation and involvement in any assembly session, activity or event. No persons or organizations other than CHA or its designated contractors may photograph, audiotape or videotape any portion of the assembly without prior written permission of CHA. CHA reserves the right to change speakers or to modify program content.

    CHA will provide an assembly registrant list to "Friends of the Assembly" sponsoring organizations. The generous support from these sponsors helps keep the registration fees low for CHA-member attendees. CHA will share only registrant names, mailing addresses and e-mail addresses (as indicated on the registration forms). Sponsors agree to use the registrant list only once.

    CHA shall have the right at its sole discretion to decline registration. If registration is declined, CHA will refund the fees paid.
